The ultimate goal of TRANShope (Helping Our People Evolve) aims to improve health outcomes for those experiencing homeliness, social isolation, loneliness and at high risk of contracting HIV/AIDS and for those newly diagnosed with HIV needing temporary housing. TRANShope was created to increase access to culturally competent care and improve health outcomes for transgender individuals while experiencing homeliness/isolation. Participants will be linked to a multiplicity of supportive services; medical, hormone replacement therapy, substance and mental health counseling, as well as trauma informed care. TRANShope is a peer lead initiative erected around the idea of formulating a communal, safe place for individuals across the gender spectrum, along with their allies to gather and be provided a supportive and welcoming environment.

The mission is to help build community among individuals who self-identify as transgender or gender non-conforming by bridging the gap among trans-positive organizations, competent health care providers and a myriad of wellness providers. AV4H officially opened the TRANShope Transitional House in July of 2020 with a ribbon cutting ceremony which gained attention from the LGBTQIA. This transitional house has been recognized as the first transitional program of its kind in the Atlanta Metropolitan area to cater to women of trans experience.

Currently this property serves as home for seven women of trans experience. To better address the needs of the trans community AV4H collaborates with THAP (Trans Housing Atlanta Program), Renaissance Kares and Unspoken Treasure Society to address the disparities which our trans community continues to encounter. TRANShope aims to reduce the stigma impacting transgender individuals, increase access to health related services through supportive partnerships.
